EMPLOYEE ASSISTANCE PLAN (EAP)

Mental health care is an important part of overall wellness. Fortunately, several area employers recognize this and offer Employee Assistance Plans, also known as, EAP's. These plans are designed to financially support and enhance employees' health and well-being. Each employer's plan is different, so Iowa Family Counseling will work with you and your employer to optimize these benefits. Together, we can improve your mental health and well-being by providing proven, effective services within the coverage of your EAP.

Initial Visit: Mental Health Assessment & Evaluation: $200
30-Minute Therapy Session: $95
45-Minute Therapy Session: $120
60-Minute Therapy Session: $170
Couples Therapy: $135
Missed Session Fee- No Show: $50+
Missed Session Fee- Late Cancellation: $25+

**ALL SELF-PAY CLIENTS ARE ENTITLED TO A GOOD-FAITH ESTIMATE OF THE COST FOR SERVICES PROVIDED. GOOD-FAITH ESTIMATES WILL BE PROVIDED PRIOR TO SERVICES BEING RENDERED.

CANCELLATION POLICY
If you do not show for your scheduled appointment and you have not notified Iowa Family Counseling at least 24 hours in advance, you may be required to pay a late cancellation fee or missed session fee.
MISSING THREE APPOINTMENTS WITHOUT NOTICE MAY RESULT IN THE PATIENT BEING
DISCHARGED AND REFERRED TO ANOTHER PRACTICE/PROVIDER.
